New Middle School Student Resources
Congratulations! We will see you in Landon's Middle School this fall!
Please download, print, fill out and return the required medical forms by August 15.
- Physician’s Examination Form -- No student will be allowed to participate in school sponsored events, athletics or activities if a physical exam on file is older than 13 months old.
- Medication Authorization Form -- required for all students who will be taking prescribed medication at school. This form is needed for students to have and carry asthma inhalers, Epipens, ADD medication, and insulin and for any prescription medication to be given at school by the school nurse or self-administered.
